Please fill out the form below as completely as you can. Unless you are requesting an IP address for a server machine, the machine name will be 'useremail.engr.rutgers.edu'.

If you do not know how to determine your Mac Address (Physical Address), click on the link and see if you can determine it. If you cannot, please fill out the form anyway and we will determine it for you at a later date.

Normally the process will take 2 or 3 days.
